Международный консенсуспо руководящим рекомендациямдля ведения пациентов с гастропатией, вызванной приемом нестероидных противовоспалительных препаратов

Аннотация

Введение. Нестероидные противовоспалительные препараты (НПВП) - одни из наиболее часто используемых препаратов в мире, которые часто связаны с нежелательными явлениями со стороны желудочно-кишечного тракта. Участковые терапевты часто сталкиваются с проблемой адекватного обезболивания с помощью НПВП и необходимостью минимизации их побочных эффектов. Это особенно важно, когда требуется длительное применение НПВП, например, у пациентов с остеоартритом и ревматоидным артритом. Чтобы помочь терапевтам более эффективно справляться с такими проблемами, группа экспертов-гастроэнтерологов собралась с целью разработки практических рекомендаций. Методы. Модифицированный процесс «Дельфи» использовали для достижения консенсуса и разработки практических рекомендаций. Двенадцать гастроэнтерологов из девяти стран предоставили свои экспертные материалы для формулирования рекомендаций. Эти рекомендации были тщательно разработаны с учетом существующих литературных данных, современной практики и мнений экспертов. Результаты. Группа экспертов разработала в общей сложности пятнадцать практических рекомендаций. Ниже приведены основные из них: НПВП должны назначаться только при необходимости; перед назначением НПВП следует оценить модифицируемые и немодифицируемые факторы риска; инфекцию H. pylori следует учитывать и лечить до назначения НПВП; пациенты должны быть ознакомлены с правилами приема НПВП; пациентам, которым требуется длительный прием НПВП, следует назначить гастропротекторный препарат, предпочтительно ингибитор протонной помпы (ИПП), и у этих пациентов необходимо тщательно контролировать любые нежелательные явления. Заключение/клиническая значимость. Эти практические рекомендации являются важным инструментом для участковых терапевтов в выборе соответствующих методов лечения своих пациентов.
